My Fiancé and I were expressing our love for one another when all of a sudden, he was bleeding down there. I freaked out even though I am undetectable. Do I have anything to worry about?

Probably not but it's best that he get tested. Is he on prep?

By all means have him go in and test. If this all occurred today he can do a 2-1-1 PrEP as a precaution. Wait a month to six weeks before he gets an HIV test, but it is quite UNlikely this caused him harm, beyond the ruptured 'roid.
