A myHIVteam Member asked a question 💭

I had vaginal sex without a condom for approximately 4 minutes. I was the insertive partner. There was no ejaculation, virtually no friction, just penetration and remaining still while she moved in a circular motion.
The woman worked at a nightclub where they perform regular health checks.
I would like to know the actual risk of HIV transmission in a situation like this.

There is a small risk of acquiring hiv, and other sexually transmitted diseases. Therefore, you should take post exposure medication within 72 hours of the incident.. but ideally within 24 hours of the incident,to prevent the possible onset of hiv.

What is the risk of HIV transmission during brief vaginal penetration without ejaculation?

There is a risk of HIV transmission, although it is lower than with ejaculation. Pre-ejaculate can also contain the virus, so transmission is possible even without ejaculation.

For vaginal sex without a condom, the estimated risk is approximately 1 transmission per 2,500 sexual acts. As the insertive partner, your risk is generally lower compared to the receptive partner, but it is not zero. Factors that influence your specific situation:

- The short duration and minimal friction may reduce the risk.
- However, any unprotected contact carries some level of risk.
- The other person's HIV status and viral load (if positive) are critical factors.
- Regular health checks are beneficial but do not guarantee complete protection.

The actual risk depends on multiple individual factors that cannot be accurately calculated for each encounter. Recommendations:

If you are concerned about this exposure, consider getting tested for HIV. It's important to know that the virus may not be detectable immediately after exposure.

Going forward, using a condom from start to finish of any sexual activity is the best way to protect yourself. You might also consider pre-exposure prophylaxis (PrEP), which reduces the risk of contracting HIV from sex by 99%.
