Having HIV is not a determining factor for employment decisions or admission to college unless the employer or college is biased and you claim a disability based on having HIV infection and perhaps seek accommodation for the disability.
The UK and US are leaders relative to HIV research and education.
Very easy to access HIV care and follow up in the US as long as you do your part to help yourself.

Mentors not that I know of? Plus, it depends on where you are going to live? In some States HIV care is great and in some States the HIV care is not good at all! I don't know how insurance works for your HIV medication or if you have any co-pay in Poland, but HIV medication in the United States is very expensive and so are co-pays! So, do your research very well and ask your doctor for all the help you will need! I'm sure that you will be able to find HIV care as well as you get in Poland.
